RÉSUMÉ Cet OFM est un numéro spécial. De fait, son éditorial est un plus court que ceux habituellement publiés dans nos numéros réguliers. Au cœur de ce numéro, nous examinons brièvement les enjeux des prochaines réunions du Comité du Fonds mondial, puis nous approfondissons certains sujets d'intérêt pour nos membres, tels que l'évaluation récente de la Politique relative aux contextes d’intervention difficiles et les défis humanitaires auxquels est confronté le Mali.

ABSTRACT This GFO editorial is a special issue and therefore a tad shorter than our regular issues. We look at the upcoming Global Fund Committee meetings and then look more i-depth into certain areas of interest to our members such as the recent evaluation of the Challenging Operating Environments Policy and event in Mali.

ABSTRACT Following on from an earlier GFO article on the appetite for risk, we argue that challenging operating environments need additional flexibilities to meet their very specific circumstances; added risk for the Global Fund, maybe, but examples from West and Central Africa show that the pay-offs can be considerable.

ABSTRACT The effectiveness of pre-exposure prophylaxis (PrEP) is well known but its availability has reached a turning point. Access to PrEP, previously solely targeted at men who have sex with men, is now being offered to women, primarily in Western countries. Some pioneering African countries are now offering PrEP to female sex workers, transgender people and the sexual partners of people who inject drugs. Three organizations in Mali, Mauritius and Morocco have developed a community project offering PrEP to HIV-exposed women. It provides women with tailored, sustainable services, combining research, advocacy and implementation. It shows real promise of success and has potential for scale up and replication.

ABSTRACT Community actors carry out many different activities in the HIV response: awareness raising and prevention activities, as well as HIV testing and referrals to health care facilities. RÉSUMÉ Cet entretien fait suite au Forum Communautaire, qui s’est tenu à Bamako du 18 au 20 janvier 2021, et qui a réuni les acteurs de la société civile malienne engagés dans la lutte contre le VIH/Sida et la tuberculose, et les représentants des Observatoires de la région d’Afrique francophone. Le Dr Bintou Dembélé, Directrice Générale d’ARCAD Santé PLUS, et Malik Samassekou, Directeur du Plaidoyer d’ARCAD, reviennent sur les enjeux et les raisons de la création d’un Observatoire au Mali.

ABSTRACT In West and Central Africa, where the COVID-19 epidemic is spreading, civil society organizations implementing Global Fund grants are organizing the community response. They are coordinating information and awareness-raising on protection, distribution of hygiene kits, and reconfiguring the available care on offer at health facilities and the protection of caregivers. They also rely on the community networks of young women and neighborhood associations.

RÉSUMÉ Reconnaissant qu’il existe des difficultés de mise en œuvre associées à une subvention de lutte contre le paludisme du Mali dont il a récemment recommandé l’approbation, le Comité d’approbation des subventions a indiqué être favorable aux plans du Secrétariat visant à « identifier les goulots d’étranglement et examiner et harmoniser globalement les mécanismes de contrôle en place, dans le but d’équilibrer atténuation des risques et mise en œuvre efficace de la subvention ». Ce souci d’équilibrer atténuation des risques et mise en œuvre de la subvention avait déjà été évoqué dans le cadre d’autres subventions.

ABSTRACT Acknowledging that there are implementation challenges associated with a Mali malaria grant that it recently recommended for approval, the Grant Approvals Committee said that it welcomed the Secretariat’s plans “to identify bottlenecks, and holistically review and harmonize the controls in place with the aim of balancing risk mitigation and effective grant implementation.” The concern about balancing risk mitigation and grant implementation has been raised before in other grants.

RÉSUMÉ Malgré leur mise en œuvre dans un contexte d'intervention difficile, les subventions du Fonds mondial ont aidé le Mali à réaliser des progrès significatifs face aux trois maladies, selon un audit mené par le Bureau de l'Inspecteur général. Il subsiste cependant des problèmes avec des péremptions de médicaments et des ruptures de stock, des retards dans la chaîne d'approvisionnement et des retards dans la fourniture (ou la non-fourniture) de tests et de traitements.
